When you submit our contact form, we collect the information you choose to provide: your name, phone number, email address, and anything you write in your message. We also use standard web analytics that collect anonymous usage data such as pages visited and general location, never your identity.
How we use it
Information you submit is used for one purpose: to respond to you about care. We do not sell, rent, or share your information with third parties for marketing. Ever.
Treatment records
