Our commitment
We are required by law to maintain the privacy of your protected health information, to provide you this notice of our legal duties and privacy practices, and to follow the terms of the notice currently in effect. Substance use treatment records receive additional protection under 42 CFR Part 2, in most cases they cannot be shared without your written consent.
How we may use your information
With appropriate consent or as permitted by law: for your treatment, to receive payment for services (such as billing the Oregon Health Plan or your insurer), and for limited health care operations like quality improvement. Other uses and disclosures require your written authorization, which you may revoke at any time.
Your rights
You have the right to inspect and receive a copy of your records, request corrections, request restrictions on certain uses, request confidential communications, receive a list of certain disclosures, and receive a paper copy of this notice.
